What the inspector wrote

On 2/23/2026 at 11:50 a.m., Licensing Program Manager (LPM), Loyce Phillips and Licensing Program Analyst (LPA), Judy Laureano conducted an unannounced Case Management visit for the purpose of serving an Immediate Exclusion for Eunice Aguilar Sanchez, an employe of this facility. Present at the facility were 6 staff and 33 children. LPM and LPA met with Director, Danielle Williams and the following was served: ORDER TO INDIVIDUAL OF IMMEDIATE EXCLUSION FROM ALL FACILITIES: Eunice Aguilar Sanchez ORDER TO LICENSEE OF IMMEDIATE EXCLUSION FROM ALL LICENSED FACILITIES: Eunice Aguilar Sanchez Appeal Rights Government Code 11522 LPM and LPA advised Facility Director that Eunice Aguilar Sanchez is excluded from this facility and any other licensed facilities by the California Department of Social Services. Eunice Aguilar Sanchez has 15 days to submit an appeal. The address for the appeal was provided to Licensee. Licensing staff served the exclusion letter to Eunice Aguilar Sanchez via Certified mail on 2/23/2026. Facility Director informed LPM and LPA that Eunice Aguilar Sanchez's last day at the facility was 1/16/2026 and has not been present at this facility since that date. Eunice Aguilar Sanchez was officially terminate on 1/30/2026 and has been removed from the Guardian facility roster as of 1/31/2026. No Deficiencies will be cited today. Notice of Site visit must remain posted for 30 days. Page 2 Exit interview was conducted with Danielle Williams, Director. A copy of the exclusion order and this report were provided to Director, whose signature on this form confirm receipt of these documents. The Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.
