What the inspector wrote

On 2/25/2026, Licensing Program Analyst (LPA) Judy Laureano made an unannounced visit at for the purpose of conducting case management inspection. LPA was greeted by facility director Danielle Williams who provided a tour of the facility. LPA observed 31 children and 6 staff members with director providing care and supervision. LPA is following up on a self-reported LIC 624-Unusual Incident Report, that was received at the El Segundo Child Care Regional Office on 1/20/2026 regarding a staff member striking a child with a shoe. Director submitted only the LIC 624. This incident required director to report it via telephone to the Department within 24 hours of happening. The director failed to report the incident to the Community Care Licensing Division within the required time frame; therefore, the facility is being cited for a Type B citation and an immediate civil penalty is being assessed. Please refer to LIC 809D. Director agrees to review Reporting Requirements regulation 101212 (d)(1)(c) Each licensee or applicant shall furnish to the Department reports as required by the Department including, but not limited to, the following: (d)Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. An exit interview was conducted, and a copy of this report, Notice of Site Visit and Appeals Rights were provided to Director Danielle Williams.
