What the inspector wrote
On 8/18/2022 Licensing Program Analyst (LPA) Judy Laureano conducted an announced inspection with applicant Nadia Barsoum for the purpose of an increase of capacity inspection of 7110 Fullbright Avenue, Winnetka, CA 91306. The purpose of this inspection is to ensure the standards for a Family Child Care Home are being met in accordance to California Tittle 22 Regulations and California Health and Safety Codes.
The licensee is applying for an increase of capacity- Large Family Child Care Home license for a max capacity of 14. Per the application, at this time, the ages the applicant wishes to provide services for children ages birth to 12 years old with the hours and days of operations being Monday- Friday from 6:00 a.m. to 9:00 p.m. Licensee was informed that any changes to ages, hours and days of operation shall be submitted to the department for approval prior to initiation of changes. Licensee has been licensed as a Small FCCH as of 3/2020.
The home is a single family unit with 4 bedrooms with 2 bathroom, living room, dining room, kitchen and play room with a \302\275 bath. LPA reviewed copy of mortage statement reflecting Nadia Barsoum and Youssef Gergis as property owner. Currently living in the home is licensee, husband Youssef Gergis and two adults children, Kerolos Gergis and Marvy Gergis.
Parents will access the home through the side entrance door. Entering the home through the side door, you are led to the play room designated as the day care room. LPA observed the room to have age appropriate toys and materials. A parent board was mounted on the side of the wall with all necessary posting. A changing table was observed next to the \302\275 bathroom located in the space. LPA observed a fire pull station next to the mounted television. Fire extinguisher and carbon monoxide and smoke detector were observed and tested.

All electrical outlets were observed to be covered. LPA observed a storage closet next to the \302\275 bath that has been designated as OFF LIMITS to the children in care. LPA observed the door to have a child proof doorknob cover. LPA observed a refrigerator next to a second exit. Licensee confirmed that program does not provide meals and/or snacks and all meals and snacks are stored in the refrigerator that are brought by the families.
The bathroom that children use is located in the daycare space. LPA observed a \302\275 bathroom, a toilet and sink area. The bathroom has 2 steps that children go up in order to use the toilet. LPA observed hand washing signage by the sink area.
Three steps that lead to the rest of the home were observed to be barricaded with a children\342\200\231s safety gate. The stairs led to the dinning room and kitchen area. LPA observed 4 pack and plays that licensee uses for napping. A working fire extinguisher and smoke and carbon monoxide detector were observed mounted in the dining room space.
The kitchen area was observed and inspected. The stove, sink and counter space area were observed and inspected. Knifes and sharp objects observed to be made inaccessible to the children in care- magnet safety latches on all cabinets. Licensee confirmed that facility does not provide meals and snacks. Licensee uses the refrigerator located in the day care room to store children's snacks and meals. LPA discuss food preparation, storage and ensuring a log and information regarding dietary restrictions and allergies are kept up to date.
The living room was observed and designated as OFF LIMITS to the children in care. LPA observed a decorative fire place that licensee confirmed is not in use. LPA requested licensee to complete a declaration-LIC 855 stating the fire place is not working and only a decorative fireplace. The living room was observed to have French doors that led to the back yard- covered patio and pool area.
Next to the living room, Bedroom 1 was observed and designated as OFF LIMITS to the children in care. Licensee confirmed that bedroom door remains closed and locked during

the hours of operations. Next to the kitchen the main entrance of the home was observed. Day care does not use the main entrance to access the home.
LPA observed a small hall way that leads to bedroom 2, bedroom 3, the master bedroom and bathroom and a full bathroom. Bedroom 2 and bedroom 3 were observed to have French doors that give access to the back yard. ALL areas have been designated as OFF LIMITS to the children in care. Licensee confirmed that all doors will remain locked during the hours of operations.
The outdoor area is access through the daycare room. LPA observed a door that leads to the back yard. Licensee walks the children using the side of the home. The designated area is enclosed and covered with artificial turf. LPA observed a variety of age appropriate materials and children\342\200\231s plastic cars and an age appropriate slide.
LPA observed the back yard to be fenced. LPA observed distinct separation between the OFF LIMITS AREA and the outdoor space of the day care.
An outdoor covered seating area and decorative fire place were observed in the space next to the designated outdoor day care space. The fire place was observed to be barricade with a glass door. The area is fenced, making it inaccessible to the children in care. LPA observed a pool at the end of the yard. LPA observed the fence to be at least five feet high and fence does not obscure the pool from view. The bottom and sides of the fence comply with Division 1, Appendix Chapter 4 of the 1994 Uniform Building Code. In addition to meeting all of the aforementioned requirements for fences, gates shall swing away from the pool, self-close and have a self-latching device located no more than six inches from the top of the gate.
Next to the pool area, LPA observed a covered deck area that has been designated as OFF LIMITS to the children in care. Children do have direct access to the covered patio, pool area or outdoor seating area with outdoor fire place.
Licensee submitted a declaration stating that both the fire place located in the living room area and the outdoor fire place are currently not in use and are decorative pieces to the home; decorative fire places.

No corrections were needed during today\342\200\231s inspections. Exit interview was conducted with licensee Nadia Barsoum. The licensing determination of this application will be reviewed with Licensing Program Manager for final resolution.

Immunizations
: Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the inspection.
Mandated Reporter Training:
Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
Volunteers are encouraged but not required to take the training.

Licensee was made reminded that it is the licensee\342\200\231s, as well as anyone who assists in providing care responsibility to know the regulations. Licensee was also encouraged to read the Child Care quarterly updates every season as they come out to stay informed of any changes or updates to statutes and regulations.
Applicant was advised that regulation prohibits the smoking of tobacco in a private residence licensed as a family childcare home during the hours of operation.
Applicant was made aware that state law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category. Applicant was also reminded that only children who are eating may be in highchairs and that car seats are utilized
only
for transportation.
Applicant was also informed that the provider is required to wash hands after every diaper change and to
never shake a baby to prevent the Shaken Baby Syndrome.

LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed.

In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification and a valid criminal record clearance associated to the facility license.

A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility.
