10:38am Licensee Johana Perez arrived at the home. LPA obtained the following documents: children's roster, children’s records, and personnel records. Additionally, LPA initiated children and staff interviews. Based on the investigation conducted, interviews of all relevant parties and record review, there is not enough information to prove or disprove that an adult in the home caused injury to child while in care, that an adult in the home hit children with an object and that Licensee is not in the home a sufficient amount of time. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED. An exit interview was conducted, and report was reviewed with Licensee Johana Perez. A copy of this report and appeal rights were discussed and left with Licensee. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 2
