What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced Required -3 year inspection to the above facility on 06/19/2025. LPAs arrived at the facility at 8:55AM, identified self and met with Susanna Bargsegyan, Licensee, who guided analyst on a tour of the facility. Licensee was provided a copy of the LIC 126 Entrance Checklist to help facilitate the inspection. Capacity as specified on the license is being maintained. LPA observed no children in care. Per Licensee, facility was closed during the inspection in observance of Juneteenth Holiday. LPA reminded Licensee to report any closures to the Department prior to the day/s of closure. During the inspection Licensee's spouse, adult son and minor child were present. Upon reviewing the Guardian Facility Roster, all adults who reside or work in the home have a criminal record clearance or exemption. LPA did not observe any excluded individuals present at this home. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The facility operates 7 days per week 24 hours per day and serve children ages 0 years to 13 years of age. ---Page 1 of 4 The family child care home is a 4 bedrooms, 2.5 bathrooms, living room, dining room, and kitchen. Current facility sketch reviewed, and Licensee confirmed that dining room, living room and 1 bathroom are used for providing care and are accessible to children. All other rooms are off-limits and made inaccessible by the use of a safety gate, and doors remain closed and locked. Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. Per Licensee, there are no firearm on the premises. LPA did not observe any items that could pose a danger if readily available to children accessible. An emergency fire/disaster drill was completed within the last 6 months. Last documented fire/disaster drill was on 05/27/2025. There is a working fire extinguisher that was purchased on 07/31/2024. LPA observed a smoke detector and carbon monoxide detector in the home that were tested and operable. LPA also observed adequate heating and ventilation for safety and comfort. LPA discussed the safe sleep regulations with licensee discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was reminded that children in care are to be supervised at all times and made aware that children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. The outdoor area accessible to children is the backyard. The back yard has a detached garage that is a guest house. Per Licensee, there is no one living in the guest house. LPA inspected the guest house and observed the guest house to be empty and not in use. There is cinder block fencing along the property line that enclosed the back yard. There is an alleyway along the backside of the back yard. There is only one gate along the property line that provides access to the alleyway and gate remains closed with a latch inaccessible to children. LPA observed no swimming pool or other bodies of water on the premises.---Page 2 of 4 LPA reviewed a sample of children\342\200\231s files and observed files were complete with LI700 Identification and Emergency information, LIC627 Consent for Emergency Medical Treatment, and LIC995 Notification of Parents Rights. Licensee has a current roster of the children. Licensee\342\200\231s Mandated Reporter Training was completed on 01/30/2024. Licensee\342\200\231s pediatric CPR/First Aid expires on 06/30/2025. Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. When a child shows signs of illness, the child shall be separated from other children and the nature of the illness determined. If it is a communicable disease, the child shall be separated from other children until the infectious stage is over. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. ---Page 3 of 4 The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and report was reviewed with Susanna Barsegyan, Licensee.
