Correction check·March 26, 2025

On 03/26/2025 at 03:15 PM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Plan of Correction (POC) Visit. LPA identified self and met with Anna Badalyan, Licensee. LPA identified reason for visit and toured the inside and outside of the facility. There were 7 children and 1 staff present. During today's visit, LPA cleared the Plan of Correction for two (2) Type B Deficiencies initially cited during a vi sit on 04/29/2025 for 102416.1(a)(10) Personnel Records and 102417(g)(8) Operation of A Family Child Care Home . LPA obtained copies of the copies of the LIC 9052 Employee Rights completed by Staff #1 and Facility Child Care Roster and provided Licensee with two (2) POC Cleared Letters. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Anna Badalyan, Licensee including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
