Routine inspection (3-year)·April 16, 2026

On 04/16/2026 at 01:00 PM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Annual Required Inspection. Facility is located at 217 N. Hollywood Way, Burbank, CA. 91505. LPA identified self and met with Jocelyn Duarte, Assistant Director. LPA identified reason for visit and provided Entrance Checklist -Child Care Centers (LIC 125) to help facilitate the inspection. Kithmini Curry, Owner/Director arrived at approximately 01:30PM. Facility Representative Jocelyn Duarte, Assistant Director allowed entry to facility and provided LPA with a guided tour of inside and outside of the facility. The facility is currently licensed for a capacity of 63, per FR they currently have 55 enrolled. The facility hours of operation are Monday through Friday from 08:00 AM to 06:00 PM. Required postings were observed near the front door facility entrance. The facility is made up of four (4) classrooms; Classroom #4 is not used. Classroom #3 ages 2 years – 3.5 years with 4 staff, Classroom #2 ages 3.5 years - 4.5 years with 3 staff, and Classroom #1 ages 4.5 years – 6 years with 2 staff. At the time of arrival, Classroom #1 and Classroom #2 were in the outdoor play yard. During the tour for Classroom #1, LPA observe Child #1 in the restroom without adult supervision in the classroom. A teacher appeared in the doorway of the classroom while the child was alone on the other side of the classroom. A Type A Citation was issued for 101229(a)(1) Responsibility for Providing Care and Supervision. Page 1 of 3 During the review of staff criminal records, LPA observed that Nayeli Maldonado did not have a criminal record clearance, exemption, or transfer an existing clearance prior to initial presence at the facility. A Type a Citation will be issue for 101170(j) Criminal Record Clearance with Civil Penalty of $500.00. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Due to time constraints, LPA was unable to complete the Required Annual Inspection and will return at a later date to complete the inspection and issue two (2) Type A Citations for sections 101229(a)(1) Responsibility for Providing Care and Supervision and 101170(j) Criminal Record Clearance.
