What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced case management inspection due to an incident that occurred on 6/11/2025. LPA arrived at the facility at 9:10 AM and met with Director, Angelica Castaneda, who guided LPA on a tour of the facility. There were 63 children and 11 staff present upon arrival. The incident that occurred on 06/11/2025, was reported to the Department on 06/11/2025, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Description of the incident: Child 1 (C1) was playing on the play yard. C1 was running in what appears to be a small hill area towards the back of the yard that goes up and down. LPA observed two dips in the hill that is made from concrete. C1 was running, fell and landed on their arm. Staff (S1) observed the child fall and took child to apply aid and get them cleaned up. S1 did not observe any injury to the child and allowed child to return to the yard. C1 went to sit on the bench and was crying and holding their arm, staff 2 (s2) went to the child to see what was wrong. Child could not move their arm, S1 and S2 took child to the Directors office. Director noticed a bone was sticking out and called 911 and their parent. C1 has not returned to the center, c1 did obtain an injury and is wearing a cast. Information reported to the Department indicated that staff did not provide adequate supervision resulting in C1 falling on the play yard and breaking a bone. LPA reviewed camera footage of the incident and observed two staff in the area where c1 was running and fell. LPA observed staff immediately go to the child to see if they were injured. Page 1. Based on interviews, and statements, and camera footage it was determined nothing could have been done to prevent Child #1 from falling and obtaining an injury. LPA advised Director to remove the hill area or place a barrier around the hill to prevent the children from having access to that area. No deficiencies were cited during today's visit. Notice of site visit must be posted for 30 days. Exit interview was conducted and report was giving to Director, Angelica Castaneda. Page 2.
