What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced case management visit at the above facility on 11/05/2025 at 12:50 PM. Upon arrival, LPA met with Director Angelica Castaneda, LPA guided herself on a tour of the facility. LPA was later joined by Licensee Kamani Goonewardena. There were 50 children with 4 staff observed present. LPA explained that the purpose of today's visit is to review camera footage for an incident that was submitted by the Director. The facility reported on 10/02/2025 that child 1 (C1) arrived at the preschool appearing normal but later became lethargic and unresponsive. Staff contacted C1's mother, and the parents picked the child up around noon. Later, the mother reported that a doctor found Tetrahydrocannabinol (THC) in child's system . According to the facility the child was only in care for 2 hours and did not eat or drink anything while present. The Department of Children and Family Services (DCFS) visited the center on October 7, and as of now, the investigation remains ongoing. No other children showed similar symptoms, and no outside food was reported that day. The child is still attending the center. The Licensee and Director showed the LPA video footage from October 1, 2025, the footage did not include audio. The LPA reviewed and recorded the portion of the footage covering the child’s time at the facility. Upon review, the child appeared energetic upon arrival but became sluggish and tired after outdoor play. LPA observed the child moving slow and constantly putting their head down on the table. LPA did not observe the child eating while in care but did observe a staff member offering the child water from their own bottle after noticing signs of fatigue. Based on review of the incident report and the video footage of the incident no deficiency was cited today. An Exit interview was conducted and a copy of this report was provided to the facility. A notice of site visit was given and must remain posted for 30 days.
