What the inspector wrote

On 5/28/25 at 8:40am Licensing Program Analysts (LPA) Jeanine Lipsey and Laticia Thompson conducted an unannounced case management licensee-initiated inspection for the purpose of a capacity increase. LPA’s met with Director Rahmath Khalid and announced the reason for the visit. The childcare center operates Monday – Friday 7AM-6:00PM. Upon touring the facility the followings issues were addressed and cited. Sheets were present on 3 of the 5 cribs that were not made for the mattress. Infant formula was seen in the cabinet where Clorox wipes, craft materiel and other objects were present. Infants had a set sleep time and were forced to lay down in the sleeping area when they were not sleep. Construction was completed in the facility and the play yard but was not reported to the department. A total of 3 walls were removed and a new play structure was installed on the play yard. The director stated they called the office, however no written documentation was submitted depicting what work was going to be done. Director stated the work was done on the week end when children were not present. Blankets, toys and sleep sacks were present in the infant cribs. Play yards were being used for infant sleeping. The infant cribs were labeled to know which infant sleeps in the cribs present. Based on LPA's observations, the following deficiencies listed on the attached LIC809D (deficiency page) are being cited in accordance with Title 22, Division 12, Chapter 3, of the California Code of Regulations. Deficiencies that are being cited need to be cleared to protect the children's health & safety. Exit interview conducted and report was reviewed with Director Rahmath Khalid. A notice of site visit was given and must remain posted for 30 days.

    Infant Care Center Sleeping Equipment(1) Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. This requirement is not met as evidence by:Based on observation, the licensee did not comply with the section sited above in that sheets on the cribs were too big for the mattress.
