What the inspector wrote

On April 11, 2025, Licensing Program Analyst (LPA) Isabel Ortega met with Director, Mary Gendler to conduct an unannounced case management inspection. The purpose of this case management is to follow up on a self reported unusual incident report (UIR) that occurred on 4/11/2025 at approximately 12:55 p.m. The unusual incident report is regarding Child #1 tripping and falling on large plastic legos and hurting arm. Upon arrival, there were 75 children in care and 13 staff proving care and supervision. During this inspection LPA completed an outdoor Health and safety observation and conducted interviews. Incident took place while Green room was outside engaged in free play. At the time of the incident there were 16 children and 2 staff providing care and supervision. According to interviews conducted, and observation conducted it has been determined the facility followed protocol. Facility applied first aid by applying an ice pack and immobilizing child #1's arm. Parent was called and child #1 was picked up and take to the medical doctor. Facility will complete the UIR within 7 days. Facility is aware incident is required to be reported to the department in 24 hours and the written report shall be submitted to the office within 7 days. According interviews supervision was active and Staff #1 and Staff #2 were outside when incident occurred providing care and supervision. Parents of child#1 involved was notified by the facility staff. Based on the interviews conducted and observation related to the incident, Facility was not cited. It was determined child#1 accidentally fell and landing on the play plastic legos child was playing with during outdoor play. A notice of site visit was provided and requested to be posted for 30 days. An exit interview was conducted, a copy of this report, notice of site visit and appeal rights were provided to facility.
