What the inspector wrote

LPA Ortega reviewed the sign in and sign out sheet for children in care showing there were 16 children present during the Green Room out door play time on 04/11/2025.  According to the staff time sheet, Staff #1 was left alone during outdoor play time when S2 went to lunch leaving S1 alone to care for 16 children.  Based on evidence obtained and interviews conducted, the allegation alleging the facility did not provide adequate supervision is deemed Substantiated.   Facility cited a Type A deficiency for lack of supervision. A second allegation was received, alleging that the facility did not report incident written report to Parent in a timely manner. During this investigation LPA Ortega conducted interviews with staff and children and collected relevant documentation regarding the above allegation.  Interviews disclosed Parent of C1 was not provided a written note of the incident on 4/11/2025 until 4/16/2025.  According to the parent handbook and Title 22 regulation this time frame is a violation of the reporting requirement.  Therefore, this allegation is deemed to be substantiated, Type B citation was issued for reporting requirements. Facility shall post the Notice of site visit for 30 consecutive days as there is immediate risk to the health and safety of personal rights of children in care.  Also, facility was informed to provide a copy of this licensing report dated 10/14/2025 documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care and to any newly enrolled parents/guardians for 12 months from the date of this report.  A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.  A finding of substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. An exit interview was conducted, a copy of this report, appeal rights and a notice of site visit were provided to facility.
