What the inspector wrote

On 10/07/2025 Licensing Program Analyst (LPA) Isabel Ortega conduct an unannounced Case Management Inspection. Upon arrival, LPA met with Director Mary Gendler, to discuss the case management finding. LPA observed 8 children present with two staff at the time of this inspection. LPA Ortega received and reviewed facility records showing the facility was out of ratio in the month of April 2025. The review revealed Staff #1 supervised 16 children when no additional staff assistance was available due to Staff #2 was out to lunch, staff #3 was off for the day and Staff #4 scheduled time started until 1:00 p.m. Based on the evidence obtained and interviews conducted, the facility is cited for Teacher -Child Ratio violation. Only one staff was present with 16 children in care. See 809 D for deficiency
