At 1:00pm Licensing Program Analyst (LPA) Lisa Clayton conducted a case management visit to follow up on an Unusual Incident Report, received on 10/19/2021. LPA was greeted by licensee Penelope "Penny" Mackie, 3 fingerprint cleared employees and 12 children in care. The home was toured for a Health and Safety inspection. LPA Clayton asked to see the area were the incident took place. Licensee confirmed that the toy (Pikler) was permanently removed from the play area and went to get it from the garage and LPA observed her take the toy out. LPA Clayton had Licensee "set the toy up" so I could take pictures of the how the toy was being used when the incident occurred (2 photos attached to LIC812). On 10/15/2021 at approximately 9:50 am C1 and another child were playing on the toy set up in the play yard. Teacher Dara was supervising one side and Licensee was on the other side watching the 2 children climb up the Pikler. C1 lost his footing, slipped and hit his mouth on the rungs of the climber. Licensee saw him slip and grabbed him immediately. Teacher Dara grabbed the other child from the climber. C1 was crying as licensee took him to the sink to rinse his mouth and see where the bleeding was coming from. Licensee called the parents and Teacher Dara called the dentist to let him know what happened, and let him know that the parents would be calling after they picked C1 up. Teacher Dara put the tooth that had come out in milk. Dad arrived approximately 10-15 minutes later. C1 was seen by a dentist and an oral surgeon and returned to child care the following Monday. LPA observed C1 in care playing with the other children and eating. LPA observed that the playground surface is maintained and in safe condition, with sufficient cushioning underneath to absorb a fall. All other playground equipment appears to be safe, in good repair and there are ample age appropriate toys. Based on interviews with licensee and staff and observations, LPA Clayton finds that there was appropriate supervision, the toy was age appropriate and being used as directed by the manufacturers instructions. There are no deficiencies cited during today's inspection. A copy of this report and notice of site visit was provided to Licensee.
