What the inspector wrote

***This is an amended report which supersedes the previous report dated 7/3/2025*** On 7/3/25, at 12PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Plan of Correction (POC) Visit to this facility. There were 49 children with 13 staff present in the Preschool Program. During a facility inspection conducted on 6/17/25, the following deficiency was issued; Type B - 101238(a) Buildings and Grounds. The Plan of Correction required the Licensee to schedule a licensed pest control company to inspect and treat the ongoing ant infestation. Additionally, the Licensee was to develop and submit a written plan outlining the approach to effectively eradicate the infestation and prevent future occurrences. The Plan of Correction was due on 6/24/25; however, the Licensee failed to meet this deadline. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Diane Connell, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

A deficiency was issued asking the Licensee to schedule a licensed pest control company to inspect and treat the ongoing ant infestation. Additionally, the Licensee was asked to develop and submit a written plan outlining the approach to effectively eradicate the infestation and prevent future occurrences. The Licensee failed to meet the POC. This poses a potential health, safety or personal rights risk to persons in care.
