What the inspector wrote
On 7/8/2021 at 4:00 PM, Licensing Program Analyst (LPA) Lillian Casillas conducted an announced second Pre-Licensing Tele-Inspection via FaceTime. LPA Casillas met with Licensee and the Licensee\342\200\231s daughter, Judith Iloulian, who helped translate for the Licensee. LPA Casillas was guided through the inside and outside of the Family Child Care Home.
LPA observed 0 adults and 0 children in care. Hours of operation are scheduled for Monday through Friday, 8:00am to 7:00pm.
Licensee is applying for a Large Family Child Care Home with maximum capacity for 14 children, ages 0 to 12 years old. Licensee is currently licensed for a Large Family Child Care Home at Iloulian Family Child Care #197402938.
This is a single-story house with 4 bedrooms, 3 bathrooms, dining room, living room, kitchen, and backyard. Childcare areas are: bedroom #1, bathroom #1, dining room, living room, and kitchen. Off-limits areas are: bedroom #2, bedroom #3, bathroom #2, bathroom #3, and backyard.
Indoor
Parents enter the property through a gate and driveway, then follow a concrete walkway to a side gate that leads to the entrance of bedroom #1. Bedroom #1 is the primary child care area. It features its own entrance and exit. In this room, LPA observed 1 couch, built-in closets lining one wall, 2 large mats, age-appropriate toys, books, and cubbies. Cleaning supplies are stored in the built-in closets and LPA observed a child safety latch on the closet door. The 2A10:BC fire extinguisher (serviced on 7/2021) is installed on the wall next to a filing cabinet where the Licensee will store the children's files and facility files. LPA observed a fire alarm and a functioning smoke/carbon monoxide detector next to the entrance door. LPA observed a First-Aid Kit located on the top shelf of a bookshelf situated next to the entrance door. A private bathroom with 1 toilet, 1 shower, and 1 sink is included in this room.

In the kitchen, LPA observed a clean and organized refrigerator. LPA observed all knives stored in an upper cabinet, making the knives inaccessible to children. LPA observed a large dining table with 8 chairs in the dining room and a china cabinet with key locks on all cabinet doors. In the living room, LPA observed 1 table with 2 chairs and a TV mounted to the wall.
Indoor Off-Limits
Bedroom #2, bedroom #3, bathroom #2 are made inaccessible to children with key locks. LPA observed child safety knobs on the doors to bedroom #4 and bathroom #3. Cleaning supplies are located in cabinets above the washer and dryer in a small room next to the kitchen.
Outdoor
LPA observed two children's benches on the front patio. Licensee stated children will eat on the benches. Children will also play in the front patio. This area is completely fenced. Licensee stated that when the weather does not permit outdoor eating, the benches will be moved inside bedroom #1. The backyard can be accessed via bedroom #1 and via sliding doors in the kitchen. The backyard includes a birdhouse room, which features a key lock, making it inaccessible to children.
Licensee stated there are no firearms or weapons of any kind in the facility at the time of the visit. LPA observed a parent board with the facility sketch, Emergency Disaster Plan, Notification of Family Child Care (PUB394), and a space for license.

SIDS & Shaken Baby Syndrome Awareness.
Licensee was reminded that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and to prevent the Shaken Baby Syndrome,
Never shake a baby!
High Chair & Car Seat Awareness.
Licensee was also reminded that only children eating may be in high chairs and that car seats are utilized only for transportation.
Parent\342\200\231s Rights Notification.
The "Notification of Parent's Rights" (PUB394) was discussed with the Licensee and the Licensee was advised that it must be posted in an area of the home accessible to parents.

Nutrition Requirement.
Licensee was made aware of AB 290, commencing January 1, 2016, requiring at least one director or teacher at each child care center or family child care home to have at least
one hour of training
in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license.
Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

Mandated Reporter Required.
Licensee was made aware of AB 1207, commencing January 1, 2018, requires all licensed providers, Licensees, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
Volunteers are encouraged but not required to take the training.

Licensing Reports Given to Parents Required.
Upon receipt by the licensee, licensees are to provide to parents/guardians the following: Copies of any licensing reports that document a Type A citation- this includes facility visits and substantiated complaint investigations;
copy of licensing documents pertaining to a conference conducted by a local licensing agency management representative and the licensee of this family child care home in which issues of noncompliance are discussed or copies of a summary of an accusation indicating the Department's intent to revoke the facility's license. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolled child at the facility.

Incidental Medical Services Awareness.
Incidental Medical Services (IMS) policy was discussed. For IMS

Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility\342\200\231s Plan of Operation and submit an updated Plan of Operation to the Department.
Incidental Medical Services Include:
Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.
An exit interview was conducted with Licensee at 4:20 PM and a copy of this report (LIC809) was sent to Licensee via email. Licensee agreed to reply to the email in lieu of a signature.
