What the inspector wrote

On 5/21/2025 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes. LPA was greeted by campus director Amanda Honda and toured the facility. LPA observed all the preschool classrooms and outdoor space. LPA observed the following: Blossom 1- 10 children and 3 staff members Blossom 2- 9 children and 2 staff members Garden 1- 8 children and 2 staff members Garden 2- 11 children and 2 staff members Campus director confirmed some of the children were out completing a community walk. LPA reviewed attendance sheets for 5/21/2025. The purpose of today\342\200\231s inspection is to follow up on an Unusual Incident Report that was reported to the El Segundo Child Care Regional Office on 4/30/2025. Unusual Incident Report states that child was left unattended in the outdoor preschool bathroom while classroom went inside for morning meeting. Child was discovered in the bathroom by a teacher. Child was left in the bathroom unattended for approximately 10 minutes. Facility contacted parents regarding incident and shared new procedures to ensure that children are always visually supervised. Page 2 During today\342\200\231s inspection LPA Laureano completed interviews with all relevant parties. Based on Unusual Incident Report received, and interviews completed, it is confirmed that facility left child without visual supervision on 4/30/2025 for approximately 10 minutes, Type A citation issued with an immediate civil penalty. Campus Director and staff confirmed new procedures in place to ensure children are supervised and accounted for at all times. Staff will be completing a census count/head count as morning meeting begins, using the attendance sheets as a reference, and when children use the outdoor preschool bathroom, a teacher will be responsible for opening and closing the bathroom door ensuring no children are left behind. Citation has been corrected. LPA cited 1 Type A deficiency during today\342\200\231s visit in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1. Please see LIC 809D. Effective January 1, 2009, all licensees must comply with Assembly Bill (AB) 978. Assembly Bill 978 requires the assessment of an immediate civil penalty for designated serious violations at community care facilities. Effective January 1, 2007, the licensee must comply with Assembly Bill 633 as follows: Copies of any licensing report that documents a Type A citation - this includes facility visits and substantiated complaint investigations. Copies of any licensing documents pertaining to a noncompliance conference between licensing management and licensees. Copies of a summary of an accusation indicating the Department\342\200\231s intent to revoke the facility\342\200\231s licenses. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolling children. The licensee shall keep verification of receipt in each child\342\200\231s file at the facility as proof of compliance (LIC 9224). Upon receipt of this report, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. An exit interview was conducted, and report was reviewed with Campus Director Amanda Honda. Copy of this report and appeal rights were discussed and left with director whose signature on this form confirm receipt of these documents.
