What the inspector wrote

On 1/5/2026 Licensing Program Analysts (LPA's), Suzette Ornelas and Nathanael Moorberry conducted an unannounced Annual Required Inspection. Upon arrival, LPA's were met by Licensees Assistant, Manushka Kojoyan who contacted licensee, ZHANNA PEPANYAN via telephone to inform her of the unannounced inspection. Licensee informed LPAs via telephone that she would return to the day care in approximately 10 minutes but stated that she did not want to allow LPAs to enter the home until she arrived. LPA Ornelas explained inspection authority regulation to licensee who agreed to allow LPA's to begin the inspection with guidance from the assistant present. LPA's got a head count of the children in care. Licensee arrived shortly after and guided LPA's on a tour of the home. Days and hours of operation are Monday through Friday 8a-5p. LPAs toured the home inside and outside and a census was taken. LPAs observed 4 day care children and 2 adults. Capacity as specified on the license is being maintained. All areas identified on the facility sketch were inspected. There are no bodies of water at the facility. Child care is provided in the dining room area as well as the dining room/living room area to the right of the home. Children also have access to the bathroom, which is located to the left of the on limits dining room. The rest of the home is off limits to children in care. The home has working telephone service and LPA confirmed the phone number is (818) 645-5444. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. LPAs observed multiple (5) knives being used for cooking on the kitchen counter. LPAs advised licensee to designate an area up high and inaccessible to children where all knives will be stored. LPAs also counseled licensee who agreed to place a baby gate and ensure that the open kitchen space will be completely inaccessible to children moving forward. LPAs observed multiple electrical outlets that were not covered (6) in the on limit areas. Licensee will ensure to place outlet covers on outlets as necessary. There is a large cabinet in the living/dining room that requires the doors to be latched due to multiple glass items inside of it. LPAs observed alcohol bottles on a shelf upon entry that need to be made inaccessible to children in care. LPAs observed a pellet gun in the licensees off limit bedroom propped up against the wall and the pellets accessible on a shelf right above the propped weapon. The First Aid Kit was observed and complete. Per LIS the facility annual fees are current. The facility roster was observed, and current. There are age appropriate toys and napping equipment on the premises. The required fire extinguisher (2A-10BC) needs to be serviced and Licensee will ensure to service yearly moving forward. Licensee will provide LPA with proof of completion via email. Carbon monoxide detectors and smoke detectors are in operable condition and tested by licensee. Facility provides daily meals for the children. Licensee has posted as required the License, and all other required postings in a visible location. Per licensee, space in the living room/dining will be used as the isolation area. Home has a fireplace in an off limits bedroom that has been screened and made inaccessible to children. There is currently 2 infants in care. LPA provided safe sleep information. Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. LPA reviewed 5 children's records and observed records were incomplete. An emergency fire/disaster drill has not been completed and documented within the last 6 months. Licensee and Assistants pediatric CPR/First Aid is not current. A review of records indicates that all employees and/or volunteers do not have immunization records on file. Licensee has not completed the Mandated Reporter Training within 2 years (2021). All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPAs and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide acces s to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 3 Type A deficiencies are being cited. This report shall be made available to the public upon request.
