What the inspector wrote

On 9/30/2021 at approximately 9:00 a.m. Licensing Program Analyst (LPA) Judy Laureano conducted an announced inspection with applicant Ghevond Melkonyan and co- applicant Anahit Hovhannisyan for the purpose of a pre-licensing inspection of 8004 Oakdale Avenue, Winnetka, CA 91306. The purpose of this inspection is to ensure the standards for a Family Child Care Home are being met in accordance to California Tittle 22 Regulations and California Health and Safety Codes. The applicants are applying for a small family childcare license with a max capacity of 8. Property owner/landlord consent (LIC 9151) is currently on file. The licensee owns the property with his wife, who does not live in the home. Narine Davtyan, wife of applicant is currently a licensed facility with facility #197493543 at 8234 Zelzah Avenue, Reseda, CA 91335. Applicant submitted a notarized declaration. Per the application, currently, the ages the applicant wishes to provide services for children 1 month to 14 years old with the hours of operation of Monday through Friday- 7:00 a.m. to 6:00 p.m. Appli cant was informed that any changes to ages, hours and days of operation shall be submitted to the department for approval prior to initiation of changes. The home is a single family home which consists of 5 bedrooms, 3 1/2 bathroom, living room, kitchen, dining room,laundry,small living room and extended family room. The home can be accessed through the screen door located on Strathem Street. The main door on Oakdale Avenue is not in use. Parents will access the daycare through the side metal gate next to the driveway on Strathem Street. LPA advised applicant to add an additional lock or latch at the top of the gate to ensure it children are not able to open it. Page 2 As you enter through the gate, LPA observed a cemented area that leads you to the main wooden door to access the day care area. As you open the door, you walk into a small living room area that has been designated for the day care. LPA observed children cubbies, children size table and chairs. A sofa was observed next to shelf that stores additional supplies for the child care. Next to the small living room/day care room, a door leads you to the family room that has been designated as a day care room. Applicant stated this room will be used for eating and activity space. LPA observed children\342\200\231s books, age appropriate toys. A television set was observed to be mounted on the wall. LPA observed a sliding door that will remain locked during the hours of operation. Applicant added an additional safety latch at the top of the sliding door to ensure children are not able to open the door. The sliding door leads you to the back yard that is OFF LIMITS to the children in care. The detached garage, small car port and extra storage shed were observed and inspected. LPA advised applicant to ensure area is locked during the hours of operations to ensure the safety of the children in care. The bathroom that children will use is located across from the sliding doors in the family room. LPA observed a toilet and sink- 1/2 bathroom. LPA advised applicant to cover the electrical outlets to ensure the safety of children in care. Applicant was able to cover the outlets during the inspection. LPA discuss handwashing procedures and COVID-19 precautions with applicant. Bedroom1 is located next to the family room. Bedroom 1 has been designated as the nap room. LPA observed 6 children\342\200\231s size bed with two cribs. LPA observed soft toys such as blocks and infant toys in the space. LPA discussed the Safe Sleep Regulations with applicant. Applicant tested the carbon monoxide and smoke detector in the space. Laundry room is located outside the small living room. LPA observed a door with a child proof door knob cover that leads to the rest of the home. Applicant confirmed that the laundry room door will remain locked at all times due to the rest of the space being OFF LIMITS to the children in care. Page 3 LPA observed the laundry room to have a Fire Extinguisher and First Aid Kit. Cleaning supplies were observed in the cabinets as well as paper goods. Outside the laundry room you are led to the kitchen, the dinning room and living room. The kitchen has a door that leads to the front yard that will not be in used. LPA observed the kitchen that is designated as OFF LIMITS. The stove, refrigerator, dishwasher, sink and counter space area were observed and inspected. Knifes and sharp object were observed to be made inaccessible to the children in care. Applicant confirmed that facility will provide meals and snacks. Dinning room and living room were observed and designated as OFF LIMITS. Through the living room you are able to access a hall way that leads you to the additional bedrooms and bathrooms. LPA observed bedroom 2 master bedrooms that include a bathroom, 2 bedrooms and a additional bathroom. The front yard has been designated as the outdoor area. Children will access the yard through the main door, they will walk down the side of the home through a second metal gate. The yard was observed to be covered with artificial turf. LPA observed a variety of age appropriate outdoor toys such as little tykes cars and plastic play houses. The yard was observed to have two trees that are enclosed with a wooden fence to ensure the safety of the children. LPA observed a covered patio that is enclosed with a wooden gate and applicant stated he will use this area for activities. LPA observed the side of the home to be safe. Applicant stated the area is off limits to the children in care. The following areas have been designated as OFF LIMITS TO THE CHILDREN IN CARE: Main door on Oakdale Avenue, Laundry Room, Kitchen, Living Room, Dinning Room, 2 Master Bedrooms with Bathrooms, additional Bathroom and two Bedrooms. Outdoors: Back yard, detached garage, small car port and storage shed. Page 4 Applicant was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State\342\200\231s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. CCAP\342\200\231s direct contact information is as followed: Phone number: (916) 654-1541 Email Address: childcareadvocatesprogram@dss.ca.gov Immunizations : Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the inspection. Mandated Reporter Training: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: www.mandatedreporterca.com . Licensee was reminded of their responsibility to report suspected child abuse. Licensee was made reminded that it is the licensee\342\200\231s, as well as anyone who assists in providing care responsibility to know the regulations. Licensee was also encouraged to read the Child Care quarterly updates every season as they come out to stay informed of any changes or updates to statutes and regulations. Applicant was advised that regulation prohibits the smoking of tobacco in a private residence licensed as a family child care home during the hours of operation. Page 5 Applicant was made aware that state law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category. Applicant was also reminded that only children who are eating may be in highchairs and that car seats are utilized only for transportation. Applicant was informed about SAFE SLEEP PRACTICES and was reminded that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome). LPA provided applicant with SAFE to SLEEP handouts. Applicant was also informed that the provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome. Individuals within one month of their 18th birthday must be fingerprinted immediately. If the aforementioned is not adhered to, a Civil Penalty of up to $500, per non-cleared adult will be assessed immediately. Please advise your analyst of any person who will be visiting regularly or for longer than one week. In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification and a valid criminal record clearance associated to the facility license. A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility. Annual fees must be paid promptly and by the due date or a late fee shall be assessed, and/or the License shall be terminated. ( If paying by check please make sure to write facility number on check to ensure that payment is applied to your facility number) The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked, and batteries should be replaced. Changes should be reported the to the Department as soon as they occur such as construction and remodeling. Telephone number changes and/or if you move from home. Reporting requirements : Applicant must report any unusual incident or injuries to the Child Care Regional office by telephone within 24 hours and in writing within 7 days. Applicant was provided with LIC 624 as a reference Fire and safety drills must be performed every six months and documented for review by the Department. (Child care Fire Drill log provided to applicant) All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation. Emergency Disaster Plan, Parent\342\200\231s Rights Poster and the Facility License are required to be posted. LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov
