Correction check·July 22, 2025

On 07/22/25, at 12:15PM, Licensing Program Analysts (LPAs) Elicia Calvillo and Daniel Bombino conducted an unannounced Plan of Correction (POC) Visit to this facility. There were 21 children with 2 staff present in the Preschool Program. During today\342\200\231s visit, LPAs cleared the citations listed below that were issued during an Annual Continuation Visit on 06/18/2025 for citations observed during an Required 3 Year Annual on 05/30/2025. Type A Citation issued for Section 101216.3(a) Limitations on Capacity and Ambulatory Status. The Director stated that they maintain the capacity that is on the facility license. Director stated they will provide the LIC9334 to LPA by the plan of correction due date 06/19/2025. LPAs conducted an in person visit on 07/22/2025 and found that the capacity of the license was in compliance with section 101216.3(a). Type B Citation issued for Sections 101217(d) Personnel Records. Director stated that the records will be updated and will email LPA a copy of the documents by the plan of correction due date of 06/25/2025. Lena Asatryan, Director provided copies of the personnel records on 06/23/2025. Type B Citation issued for 101212(b) Reporting Requirements. Director stated they will complete LIC 308 Designation of Facility Responsibility and provided supporting documentation and email to LPA by the plan of PAGE 1 OF 2 correction due date 06/25/2025. Lena Asatryan, Director provided copies of LIC 308 Designation of Facility Responsibility and provided LPA with a copy of the form. Type B Citation issued for 101238.2(d)(2) Outdoor Activity Space. Director stated that they will ensure the play yard is free of hazards and will create a daily facility log and provide LPA a copy of the log by the plan of correction 06/25/2025. LPAs toured the outside play area and did not find any hazards in the play area. Lena Asatryan, Director stated that they are still planning the daily facility log. LPAs cleared the plan of correction for the sections listed above and provided Lena Asatryan, Director with the Plan of Correction Letter. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Lena Asatryan, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. PAGE 2 OF 2
