On 09/16/2025 at 10:00 AM, Licensing Program Analyst (LPA) Elicia Calvillo, Licensing Program Manager (LPM) Rita Ramos, and Regional Manager (RM) Claudia Guangorena conducted an unannounced Case Management -Other Visit at the facility. LPA, LPM, and RM identified self and met with Lena Asatryan, Director who guided LPA, LPM and RM on a tour of the inside and outside of the facility. LPA, LPM, and RM observed 26 Children and 4 staff at the facility upon arrival. The purpose of the visit is to discuss the pending capacity increase request and to obtain signatures on an amended report dated 08/22/2025. The following items need to be completed. Current final approved Certificate of Occupancy A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Lena Asatryan including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role
