What the inspector wrote

On 01/25/2022 at 8:00 am Licensing Program Analyst (LPA) Deborah Lowe met with applicant Tara Perera. The purpose of the visit was a second announce pre-licensing inspection to review for a change in ownership application at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 11 children present during this inspection. This is an application for 27 infants, ages 0 months to 24 months. The child care center will operate Monday \342\200\223 Friday 7:00 a.m. \342\200\223 6:00 p.m. Applicant has a pending application for a Preschool License #197494946. The fire clearance was approved on 10/22/2021. The 1st pre-licensing visit was conducted on 01/03/2022. LPA Lowe is conducting a 2 nd pre-licensing visit to evaluate the facility corrections needed before a license application will be considered. Deep cleaning to classrooms including tables and walls. LPA Lowe observed the area to been clean. Repair tile flooring in room 2. LPA Lowe observed tile is still in need of repair. Remove indoor climber from room 2, observed to be for ages 2 years \342\200\223 6 years old. LPA Lowe observed the indoor climber has been removed from room 2. Provide lids to two containers used for storage of solid waste in room 1. LPA Lowe observed one storage of solid waste container with a secure lid, one solid waste container was observed in need of a lid. Remove pesticides from kitchen area, ensure poisons are locked and not stored in a kitchen area or food-preparation area. LPA Lowe observed no poisons or pesticides in the kitchen area. Provide disaster and mass casualty plan of action. Applicant stated will complete and send to LPA Lowe. Repair or replace cribs \342\200\223 LPA observed cribs to be in good repair. Organize crib room to ensure staff accessibility to all cribs. LPA observed crib area to be organized with accessibility to all cribs. Ensure crib area is physically separate from indoor activity space. LPA Lowe observed a gate separating the crib area from the activity area. Remove toys and infant dishes from changing table sink. LPA observed sink to be clean and free from dishes and toys. Provide a crib or playpen for isolation area. LPA Lowe observed a napping mat in the directors office. Applicant stated Directors office will be used as isolation area. Secure shelving units to the wall. LPA observed shelving units to be secured to wall with use of bolts and earthquake straps. Remove or replace broken outdoor play equipment and torn sand backs from playground #1. LPA observed broken play equipment and sandbags to have been removed from playground #1. Remove metal spring rider animal shape riders from playground #1. LPA Lowe observed spring riders in place. LPA Lowe requested a plan to ensure child safety while using playground #1. Provide updated facility sketch with room numbers and playground numbers. Applicant stated they will send updated sketch to LPA. Provide safety outlet covers to exposed outlets. LPA Lowe observed safety outlet covers on all outlets. Make kitchen drawers and cabinets containing hazards and sharps inaccessible to children. The kitchen has been made inaccessible with the use of a locked door. Provide potty chairs for potty training infants. Applicant stated they have ordered. Applicant stated they will send LPA the invoice showing potty chairs have been ordered. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. The following corrections are still needed before application will be considered, corrections due by 01/31/2022: Repair tile flooring in room 2 Provide lid to one container used for storage of solid waste in room 1. Provide disaster and mass casualty plan of action. Provide updated facility sketch with room numbers and playground numbers. Provide potty chairs for potty training infants. Send LPA invoice showing proof of purchase with estimated date of delivery. LPA Lowe observed spring riders in place. LPA Lowe requested a plan to ensure child safety while using playground #1. Request for outdoor waiver to share playground #1 with preschool. Include written request, outdoor schedule, and individual classroom schedules. Exit interview conducted and report was reviewed with applicant Tara Perera.
