(a) The department may prohibit any person from being a member of the board of directors, an executive director, or an officer of a licensee or a licensee from employing, or continuing the employment of, or allowing in a licensed facility, or allowing contact with clients of a licensed facility by, any employee, prospective employee, or person who is not a client who has: (2) Engaged in conduct that is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility, or the people of the State of California. This requirement was not met as evidenced by Director finding a controlled substance placed in the bathroom by S2 at the facility. This poses an immediate health and safety risk to children in care.

SIGN IN AND SIGN OUT

(b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement was not met as evidenced by: LPA observation of time sheets from 6/1-8/13/25 with multiple staff checking children in/out on behalf of the parent/guardian. This poses an potential healthy and safety risk to children in care.

Reporting Requirements-Each licensee or applicant shall furnish to the Department reports as required by the Department including, but not limited to, the following: ... (d) report as required on the (1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION
