What the inspector wrote

On 02/17/2022 at 8:30 am Licensing Program Analyst (LPA) Deborah Lowe met with Applicants, Arshaluys Indzhyn and Mariam Khachatryan. The purpose of the visit was a second announce pre-licensing inspection to review for a change in ownership application at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. This is an application is a change of ownership for 40 children, ages 2 years to entry into 1 st grade. The child care center will operate Monday \342\200\223 Friday 7:00 a.m. \342\200\223 5:30 p.m. The fire clearance was approved on 12/10/2021. Applicant has a pending applicant for an Infant License #197494974. LPA Lowe observed 27 children present under direct supervision of 4 staff during this inspection. Staff present were observed to have their criminal record clearances. The 1st pre-licensing visit was conducted on 01/28/2022. LPA Lowe is conducting a 2 nd pre-licensing visit to evaluate the facility corrections needed before a license application will be considered. Make circular metal items in ground near air conditioning unit on playground inaccessible to children. \342\200\223 LPA Lowe observed the circular metal items in ground near air conditioning unit to have been removed, no hazards visible. Repair or make a plan for access gate from walk way to playground \342\200\223 to ensure the safety of the children. Gate was observed to not close all the way on its own. \342\200\223 LPA Lowe observed the gate has been repaired and LPA observed the gate close on its own without assistance. Verification of appropriate age usage of play equipment on preschool playground. \342\200\223 LPA Lowe observed the play equipment that verification of appropriate age usage was requested was removed from facility instead. Facility Sketch update to include 2 nd floor rooms and off limit areas. \342\200\223 Applicants provided LPA an updated facility sketch for the 2 nd floor to show off limit areas to facility and children. Repair tile floor at entrance to outdoor bathroom \342\200\223 LPA Lowe observed the tile floor located at the entrance to the outdoor bathroom to have been repaired. Floor tile was observed to be in good repair. Repair counter tile in room 1B. \342\200\223 LPA Lowe observed the counter tile in room 1B to have been repaired with no exposed broken pieces. Provide mass casualty plan of action. - LPA Lowe reviewed facilities mass casualty plan. LPA advised facility to keep mass casualty plan available to be reviewed during future facility visits when requested. The following items were discussed with applicants: Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. An exit interview was conducted with the above items discussed and a copy of this report was provided to the Applicants, Arshaluys Indzhyn, Mariam Khachatryan, and Director, Jessica Torres. Final license determination will be made upon review by the Licensing Program Manager.
