What the inspector wrote

Page 2 Based on observation, staff interviews, document review and LPA\342\200\231s observations, facility was operating out of ratio on 5/28/2025. LPA Laureano was also able to confirm that facility transported children without seat belts on 5/21/2025. he preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. LPA Laureano informed facility representative Perla Venega that this report dated 5/28/2025 documents 2 Type A citations which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Judy Laureano informed facility representative to provide a copy of this licensing report dated 5/28/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Upon on receipt of this report, the facility director shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. An exit interview was conducted, and report was reviewed with Facility Director Perla Venega. Copy of this report with copy of Appeal Rights were provided and left with Licensee, whose signature on this form confirm receipt of these documents.
