What the inspector wrote

On 2/2/2022 at 9:30am Licensing Program Analyst (LPA), Loyce Phillips conducted an announced Pre-Licensing inspection with applicant Anair Asatryan. Today's inspection is a follow up to the Pre-Licensing inspection conducted on 1/25/2022. During 1/25/2022 Pre-Licensing appointment, applicant stated the backyard and right side of the outdoor area was off limits. On 2/2/2022, applicant wishes to make the back yard and right side of the outdoor area accessible to children. The back yard is fenced, with artificial grass and has a medium slide/swing set. The far left of the backyard is fenced and off limits to children. LPA observed a small empty room. Applicant stated it will be a 4th bathroom. The bathroom is located in back yard adjacent to the house. The bathroom can only be access from outside. Applicant stated she does not have a building permit for the outdoor bathroom. The bathroom is off limits to children. LPA requested an updated facility sketch reflecting this addition. The following corrections were required on 1/25/2022: * Make kitchen inaccessible * Complete parent board * Make Fire place inaccessible During today inspection the following corrections were observed and completed: * Kitchen is inaccessible by child safety gate. * Parent board is complete located by front entry way. * Fire place is inaccessible by safety latch. An exit interview was conducted and a copy of this report was provided to the applicant.
