What the inspector wrote

On 11/10/2025 Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced case management inspection at the above-referenced facility. The purpose of the inspection is to ensure that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes. At the time of inspection from 8:20am to 9:05am, eight (8) children were present in Preschool Classroom No. 2 under the supervision of one staff member (S3), who holds the position of aide. At approximately 8:33 AM, one toddler (C1), aged 18 months, was dropped off in the same classroom. The oldest child present in the classroom is five years and ten months old. The designated toddler teacher (S1) arrived at the facility around 9:00 AM. Licensee Cherelle Rivera arrived at the facility at approximately 12:35pm. The Licensing Program Analyst (LPA) reviewed the following documents: Personnel records, children’s records and the children’s sign-in/sign-out sheets for the month of November 2025. LPA’s observation and record review revealed that Staff member S3, who was providing care and supervision during the inspection, does not meet the qualifications of a teacher. The facility was operating out of ratio, as the presence of only one unqualified aide supervising eight children does not meet regulatory standards. Staff were reminded that per licensing regulations, a teacher aide may be used in a ratio of one teacher and one aide for every 15 children, but not as the sole caregiver. Continue Based on observations, record reviews, and staff interviews, the facility was found to be out of compliance due to the presence of an unqualified staff member providing care. There were two (2) deficiencies cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information. Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit. An exit interview was conducted, and Plan of Corrections were reviewed and developed with Licensee Cherelle Rivera. A copy of this report and appeal rights were discussed and left with the Cherelle Rivera, whose signature on this form confirm receipt of these documents. Page 2
