Inspector’s narrative

What the inspector wrote

Based on LPA observations, interviews of all relevant parties and record review, no information revealed that staff used an inappropriate form of punishment on a day care child and that staff did not allow a day care child to use the restroom when needed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED. An exit interview was conducted with S3. A copy of this report and appeal rights were discussed and left with S3. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 2
