Inspector’s narrative

What the inspector wrote

On 12/15/2023, LPA Risher conducted an interview with the Licensee over the phone. LPA requested a copy of the facility roster. On 01/18/2024, LPA Risher conducted interviews with staff and children. On 02/15/2024, LPA Risher conducted additional interviews with children. Staff conduct a well-ness check upon arrival for each child. Staff document their observations in the Brightwheel app. Staff will discuss any concerns with parents. Staff will complete an ouch report if they observe injuries on the child. The facility has not had any outbreaks or illnesses. Staff will notify parents when the facility is closed due to an outbreak. Staff will discuss signs of illness with parents when needed. Staff will notify parents when they observe injuries on the children in care. There has not been any incidents at the daycare that required the staff to seek medical attention. LPA observed the licensee and 3 staff providing care to the daycare children. The staff present have a fingerprint clearance on file. Parents and children identified the staff that provide care. The individuals names provided match the names associated to the facility. There are at least 2 staff present during operating hours. There was 1 staff that stopped working for the daycare in 2023. However, Licensee has at least 2 assistants present along with the Licensee at the facility throughout the operating hours. A full investigation was conducted which including observations and interviews. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the above alleged violations did or did not occur, therefore the allegations of Personal Rights, Ratio, Reporting Requirements and Qualifications are found to be unsubstantiated. Exit interview was conducted with Tabina Dixon, Licensee. Appeal rights was provided to Licensee.
