What the inspector wrote

On 3/27/2026, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Director Meri Movsisyan who guided LPA on a tour of the facility. LPA explained the purpose of today's visit to follow-up on an incident which occurred on 3/16/2026. The Licensee Representative / Bhagya Fernando , arrived shortly after. There were 30 children with 5 staff observed at the time of visit. The incident that occurred on 03/16/2026, was reported to the Department on 03/16/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The written report has been received to the Regional Office. The incident which occurred, Child #1 had a seizure while in care. Child #1 was present at the time of the visit. LPA collected children's roster, conducted interviews with staff, took photos of where the incident occurred, and obtained other pertinent information. Page 1 of 2 During staff interviews LPA was informed Child #1 was walking when Child #1 tripped forward and bumped head on apparatus. There was nothing in Child #1 path that caused them to trip. Staff #1 was their when child stumbled backwards after bumping their head. Staff #1 was able to place Child #1 on the ground, Child #1 began to seize During that time children were taken inside to the classroom. Staff #1 informed LPA, that they were advised by 911 operator to place child on their side. Parent arrived first to the facility, Ambulance arrived shortly after. Child #1 was taken to the hospital. While at the hospital, Child #1 was cleared by Doctors to return to school with no restrictions. LPA Morales spoke with Parent #1 regarding the incident, Parent #1 has no concerns. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days.
