Inspector’s narrative

What the inspector wrote

On 6/4/24, at 11AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an announced follow up capacity increase / Annual Random Inspection. The purpose of this visit is to ensure the corrections requested during the visit conducted on 5/14/24 had been completed by the Licensee. Licensee has submitted a capacity increase application for a large family child care license with up to 14 children - ages 1 month - 13 years of age. The facility\342\200\231s hours of operation are 8AM - 11:59PM, Monday - Friday. Upon arrival, LPA met with Licensee / Azatuhi Avetisyan, who guided LPA on a tour of the inside and outside of the home. LPA observed 7 children in care, of which 2 are infants. Licensee has designated the artificial grass area located in front of the facility/next to the garage, as on limits (play area) for children in care. During today's visit, LPA observed the designated play area was covered with artificial grass and fully enclosed with small fencing, making the large bricks and trees/shrubs around the front yard inaccessible to children. During today's visit, LPA observed Licensee had placed wooden fencing inside the designated play area, making the Gorilla Skyline Lookout Outdoor Playset inaccessible to children under the age of three (3) years old. Licensee understands that children will not play outside of the designated play area as it can be a hazard due to the bricks and trees/shrubs being accessible to children. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted and a copy of this report, along with the appeals rights were issued to the Licensee. Final capacity increase determination will be made upon review by the Licensing Program Manager. There were no corrections requested during this visit.
