What the inspector wrote

On 03/21/2024 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced case management inspection. During the inspection, LPA Laureano observed on child (19month old) napping in classroom 2 with 6 other preschool children. Director confirmed that the toddler component only has 1 child enrolled. LPA observed the toddler classroom. Director moved child over to the toddler classroom and stated that director is providing care and supervision to this one toddler. LPA issued a Type A citation for having toddler child napping in the preschool room. Facility was cited under 101216.4 (a)(2). Please see LIC 809D. Exit interview conducted and report with appeal rights was reviewed with the director Asma Siddiqui. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not being met as evidenced by: On 3/21/24, at approximately 12:28PM, LPA observed 2 children in the bathroom while staff supervised 10 napping children in the classroom. LPA observed 7 napping children in class room 2, unsupervised, while staff walked over to the ktichen area. This poses an immediate health, safety, and personal rights risk to children in care.

    101238 Buildings and Grounds(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requierment was not being met as by: Classroom 2 was observed 4 large screws protruding from the wall near two children’s crib. Cribs are not currently being used by children. LPA observed 7 children napping in cots in the same classroom.

    101216.4 (a) (2) Preschool Program with Toddler Component (a) Licensees serving preschool-age children may create a special program component for children who are between 18 months and 36 months of age ...(2) The toddler program shall be conducted in areas physically separate from those used ...Plans to alternate use of outdoor play space must be approved by the Department. This requirement was not being met as evidenced by: On 3/21/24 while children were napping , LPA observed a 19 month old child napping in the preschool room with 6 preschool children. This poses an immediate health, safety, and personal rights risk to children in care.
