Correction check·March 13, 2025

On 03/13/2025, Licensing Program Analyst (LPA) Adrian Risher conducted a case management Plan of Correction visit for a citation issued on 03/06/2025. LPA met with Viviana Salas, Site Supervisor and provided the purpose of the visit. LPA observed 13 children in the yellow room with 2 staff and 5 children in the toddler outside play area with 1 staff. Madison Sanello, Director arrived around 10:50am while LPA was conducting the inspection. On 03/06/2025, the facility was issued a Type A violation for Limitations on Capacity. The facility was caring for children under 2 years of age prior to obtaining the license and fire clearance. Director Madison Sanello and Site Supervisor Viviana Salas were notified to cease enrollment of the children under the age of 2 years old. The plan of correction was due by 3/10/2025. On 3/13/2025, LPA observed 2 children under the age of 2 years old in the outside play area with 1 staff. The facility has failed to correct the deficiency issued on 3/6/2025 for section code 101161 Limitations of Capacity by the 3/10/2025 due date. This is an immediate health and safety risk to the children in care. This is a type A violation under Health & Safety Code 1596.99. The facility is being issued a civil penalty for $100/per day due to the a failure to correct the plan of correction that was due on 3/10/2025. LPA did not observe the notice of site visit from the inspection conducted on 3/6/2025 posted on the parent board. This is a potential risk to the health and safety to the children in care. The facility failed to maintain the Notice of Site Visit posting for 30 consecutive days and will be issued a $100 civil penalty. This is a type B violation under Health & Safety Code 1596.8595. This poses a potential risk to the health & safety of the children in care. LPA reviewed children\342\200\231s files and did not observe the LIC9224. Site Supervisor stated that the LIC809 Facility Evaluation Report dated 3/6/2025 was not given out to the parents since the decision was under appeal. This is a type B violation under Health & safety Code 1596.8595. This poses a potential risk to the health & safety of the children in care Upon receipt of this report, the facility representative shall post the Notice of Site Visit (LIC 9213) and any Licensing report documenting a type \342\200\234A\342\200\235 deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224) form must be maintained in each child\342\200\231s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224). Exit interview was conducted and a copy of the report was provided.
