What the inspector wrote

On 7/17/25, Licensing Program Analyst (LPA), Veronica Wheatley met with the Executive Director Nicole Royston and Community Coordinator Melissa Kringen-Kimble to conduct a Case Management Inspection due to an incident that occurred on July 10, 2025 whereby Child #1 wandered out of the facility and passed the parking lot to the sidewalk and was found safe by two passers-by. Today, LPA Wheatley observed 43 children in care with proper teacher/child ratios. During the investigation LPA interviewed 5 staff members to obtain additional information. According to the staff, all of the children were playing on the playground and supervised by required staff. LPA verified with attendance sheet that there were 43 children present the day of the incident. The incident was reported to the Regional Office in a timely manner. The report was sent the following week. The facility was cited a Type A deficiency according to California Code of Regulations Title 22 for Lack of Care and Supervision. (See LIC 809D report for deficiencies). Exit interview conducted. A copy of this report was read and provided to the Executive Director.

This requirement was not met as evidenced by Child #1 was found wandering outside of the facility. Staff admitted that Child #1 was found outside the facility, pass the parking lot and returned by two passerby. Child was found safe. This poses an immediate risk to the health, safety, or personal rights of children in care.
