What the inspector wrote

On 08/4/2025 Licensing Program Analysts (LPA)s Judy Laureano and Cristina Castellanos conducted an unannounced Case Management \342\200\223 Plan of Correction inspection at the above mentioned facility. The inspection was conducted to ensure the licensee is in compliance with Title 22 regulations related to deficiencies cited on 7/17/2025. LPA Laureano and LPA Castellanos were greeted by Facility Director Tina Avila and completed a walk through of the facility. LPAs did not observed any children in care due to school being close for staff development day. LPA Laureano inspected and observed the following: The two gates facing 10 th Street- facility has added battery operated alarms that alerts staff when gate is opened. LPAs observed an added safety latch on both gates. Middle Gate that leads to the playground \342\200\223 top latch was added as well as the gate remains locked. Key is kept near the door at all times only accessible to staff. Playground Gates facing Rowell Avenue- facility has added a battery operated alarm as well as safety latch on the top of the gate. . Facility Gates facing Rowell Avenue- battery operated alarms on gates. Page 2 The director stated that gate alarms are checked daily by staff as part of their Playground Outdoor Safety Checklist. LPAs received and reviewed Playground Outdoor Safety Checklist (Before Children Go Outside) where safety checks for gates is mentioned multiple times, declaration ensuring children\342\200\231s safety (LIC855),retraining of teachers regarding care and supervision and yard plan and other additional documentation requested. Citations issued on 7/17/2025- Lack of Care and Supervision has been cleared. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies or violations were cited during today's inspection. An exit interview was conducted, and this report was reviewed and provided to Facility Director Tina Avila. A notice of site visit was provided which must remain posted for 30 days.
