What the inspector wrote

On 08/04/2025 Licensing Program Analysts (LPA) Cristina Castellanos and Judy Laureano conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes. Upon arrival, LPAs were greeted by Jessie Miller, Director and shortly after were met by Tina Avila, Facility Director. No children were present during today\342\200\231s inspection. Director Avila stated the facility is closed for Professional Development in preparation for the new school year. On 07/23/2025, officer of the day Licensed Program Analyst (LPA) A. Risher received a call from the facility in connection with an Unusual Incident that occurred at the facility in regard to a child 1 (C1) rolling her right ankle as she walked on the play structure and had a misstep. C1\342\200\231s parent took her to urgent care were it was determined that C1 fractured her right tibia. The doctor advised C1 to rest for a few days before returning to school. Per Director Avila C1 will return for the new school year on 08/18/2025. During today's visit LPAs observed and inspected the area where the incident occurred. Based on the information obtained, interview conducted and observations of the play structure C1 missed a step which caused an injury to their right ankle. Since the incident, as an extra precaution the facility has placed anti-slip tape - yellow and black - all around the play structure to distinguish steps and levels when moving around the apparatus. At this time no further investigation is needed. According to Title 22 and the Health & Safety Code no violations have occurred, and no deficiencies were cited. A Notice of Site Visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Director, Tina Avila .
