Inspector’s narrative

What the inspector wrote

The Department conducted a full investigation, which included staff interviews, interviews with relevant parties and other agencies, as well as a record review which included documentation related to the allegations. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Personal Rights- Staff did not meet day care child\342\200\231s toileting needs and Staff expose daycare children to hazards. During classroom observations LPA Whitmore observed children\342\200\231s toileting needs being met by changing diapers and documenting information. During observations in the classroom LPA did not observe any hazards that were exposed to the children inside and outside the facility. Physical Plant- Staff did not ensure that the facility is free from pests and Staff did not ensure the facility is clean. During classroom observations all three classrooms were cleaned, and LPA did not observe any pest. LPA conducted interviews and classroom observations. Therefore, the allegations are deemed unsubstantiated. Meaning, although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. No deficiencies cited. An exit interview was conducted, copy of this report was read, appeal rights along with Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days.
