What the inspector wrote

On 01/02/2025 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegation. LPA was greeted by Director Elizabeth Cooper. During the course of the investigation LPA requested and reviewed the following documents: children's roster, children\342\200\231s records, staff roster, personnel records, children\342\200\231s sign-in/sign-out sheets for the month of December 2024, infant sleep observation forms-December 2024, staff timesheets for the month of December 2024, the parent handbook, a copy of the Baby Bear/Little Cub Daily Report and a copy of the Ouch Report. On 03/20/2025 LPA concluded all interviews of all relevant parties. Based on LPA observations, interviews of all relevant parties and record review, no information was disclosed that children\342\200\231s personal rights were violated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED. An exit interview was conducted with Director E. Moore. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 2
