What the inspector wrote

On 03/04/2026 Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named facility to conduct a Plan of Correction visit and was met by Lead Teacher Frankie Hernandez . LPA observed 38 children being cared for and supervised by six fingerprint cleared staff. On 09/17/2025, the facility was cited for the following: 1. Director will ensure that all staff complete mandated reporter training. Director will ensure that completion certificates for mandated reporter training are in staff files for review. 2. Director will ensure all staff are up to date on immunizations. Director will ensure that staff files are updated to show all immunizations required for childcare providers. 3. Director will ensure staff compete health screening including TB test. 4. Director will ensure all staff, enrichment coordinators, and volunteers are fingerprint cleared and associated to the facility prior to working and or volunteering. 5. Director will ensure all staff and children will maintain a visual supervision rate of 12:1 at all times. During today\342\200\231s visit licensing staff observed the following: 1. Missing Mandated reporter completion certificates in staff files. Not cleared on today\342\200\231s visit. 2. Missing staff immunizations on file for all staff including flu vaccine/flu declinations, mmr, tdap. LPA reviewed staff immunizations on 03/04/26. Not cleared on today\342\200\231s visit. 3. Missing TB tests in staff files. Not cleared on today's visit. 4. Enrichment coordinator on site for enrichment activities today. Enrichment coordinator not associated with CCC. Not cleared on today\342\200\231s visit. 5. During outdoor play, LPA observed Lion classroom on the playground. Staff maintained a 12:1 ratio with children during LPA observation. Cleared on today\342\200\231s visit 1 of 5 Citations issued on 9/17/2025 has been cleared. An exit interview was conducted. A copy of this report, notice of site visit, and deficiencies clearance letter were discussed and provided to Director Mahlia Fakhri .

LPA observed fingerprint cleared staff not associated with CCC and providing care to children.

    (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (12) Tuberculosis test documents as specified in Section 101216(g). This requirement is not met as evidenced by:LPA reviewed staff files. LPA observed staff file missing TB test administration and results.

  • ARTICLE 1. General Provisions and Definitions

    (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.... influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by:LPA reviewed staff files. LPA observed three staff files missing staff immunizations.

  • ARTICLE 2. Administration of Child Day Care Licensing

    (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider... shall complete the mandated reporter training.. (a), and shall complete renewal mandated reporter training every two years This requirement is not met as evidenced by:LPA reviewed staff files. LPA observed three staff files with expired mandated reporter training certificates.
