What the inspector wrote

On 04/03/2025, Licensing Program Analyst (LPA) Tyra Chavies  conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 03/22/2025. Upon arrival, LPA met with Assistant Director, Aisha King.  LPA informed  assistant director about the purpose of the visit and toured the facility. LPA observed 60 children being supervised by 7 teacher. Incident Details: Child #1 was playing on the yard with another child #2 next to soccer goal and had a conflict. Child #2 pushed child #1 and causing Child #1 hit his eyebrow and cheek onto the soccer goal post. The child sustained an injury. On 03/14/2025 LPA Chavies conducted phone interview with Director On 03/27/2025 LPA Chavies observed children in care On 04/03/2025 LPA Chavies observed children in care, received facility and personnel documents, interviewed S #1, S #2 and P#1 Exit interview conducted and copy of report and appeal rights were reviewed  and provided to Director,  Demi Lara. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, Technical Violation was given today. See LIC 9102. A Notice of Site Visit was provided and must remain posted for 30 days.
