What the inspector wrote

On 10/07/2025, Licensing Program Analyst (LPA) Tyra Chavies  conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 09/04/2025. Upon arrival, LPA met with Director, Demi Lara.  LPA informed director about the purpose of the visit and toured the facility. LPA observed 119 children being supervised by 14 teachers. Incident Details: RP states...  dad reported that during evening bath time mom noticed a scratch on C1 back (from top to bottom) and inner thigh (right). Per director, there was no report from teachers regarding incidents to C1 . On 09/26/2025 LPA Chavies conducted phone interview with P#1 On 09/22/2025 LPA Chavies conducted in person interviews with Director, S#1 and S#2 On 09/22/2025 LPA Chavies toured the facility inside and outside, observed children in care, received facility and personnel documents Based on observation, interviews conducted and review on documentation, once the facility was notified of the injury, the facility followed Title 22 California Code of Regulations and the Health and Safety Codes governing California. No deficiencies were cited. Exit interview was conducted and a copy of the report was provided to director, Demi Lara. Notice of site visit was given and must remain posted for 30 days
