What the inspector wrote

On 03/11/2026 LPA'S Doris Whitmore and Patsy Plancarte arrived at the facility for a Case Management Inspection. LPA'S verified the Guardian Roster dated 03/09/2026. Staff. does not have a cleared criminal record clearance to be present at the facility Based on interviews with the licensee it was confirmed that this staff l has been present at the facility for the last three days( 3/09/ 2026 3/10/2026. and 3/11/2026). LPA 'S cited a Type A citation, please see Lic 809 D. Civil Penalty will be assessed. LPA'S informed licensee Darlene Garner that this report dated 03/11/2026 documents one Type A citation Type A citation(s) which shall be posted 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA 'S informed licensee Darlene Gardner to provide a copy of this licensing report dated 03/11/2026 that documents any Type A citations(s) to parents/guardians of all children currently enrolled by the next business day or the next day or the next day children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report ( LIC9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the licensee Darlene Gardner
