What the inspector wrote

On 3/28/2025, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on an Unusual Incident, reported to the department by telephone on 3/19/2025. LPA was greeted by Owner/Licensee, Diedre Nordel. LPA toured the facility and took a census of the children. LPA observed a total of 78 children in care with 19 staff members. Description of the incident : On 3/19/2025 it was reported anonymously to LPA that on 3/18/2025, Staff 1 (S1) and Staff 2 (S2) were lining their class to transition outside. Child 1 (C1) was standing near the staff break room door. C1 fell and was trying to pull himself up, placing his hand inside the hinged side of the door. Staff 3 (S3) was exiting the break room and did not notice C1 hand inside the door. S3 closed the door while C1 hand was still placed in the door causing bruising. First aid was applied, parent was called, and staff was written up. During this inspection, LPA toured the facility and interviewed staff. Based on the information provided and interviews conducted further investigation is required. An exit interview was conducted, a copy of this report and notice of site visit was provided to Owner/Licensee, Deidre Nordel.
