On 3/28/2025, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on an Unusual Incident, reported to the Department on 2/24/2025 . LPA was greeted by Owner/Licensee, Diedre Nordel. LPA toured the facility and took a census of the children. LPA observed a total of 78 children in care with 19 staff members. At 9:45am, LPA participated in the emergency fire drill. Description of the incident : On 2/19/2025, during nap time between 12:30 to 2:00pm, parent of C1 arrived to the facility to pick child up. Parent was at the door when she observed Staff 1 (S1) physically picking her child up and forcefully placing him on his mat. S1 stated child was on his knees and she placed him on his mat. On 2/20/2025, Parent spoke with Director about what she observed. Director, stated she would do an internal investigation and follow-up with parent regarding the incident. During this inspection, LPA toured the facility, interviewed staff and obtained pertinent information related to this unusual incident. Based on the information provided and interviews conducted further investigation is required. An exit interview was conducted, a conducted, a copy of this report and notice of site visit was provided to Owner/Licensee.
