What the inspector wrote

On 4/25/2025, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on and close out the Unusual Incident, reported to the department on 3/19/2025. LPA was greeted by Director, Telma Rojas and toured the facility and took a census of the children. LPA observed a total of 81 children in care with 19 staff members. Description of the incident : On 3/19/2025 it was reported anonymously to LPA that on 3/18/2025, Staff 1 (S1) and Staff 2 (S2) were lining their class to transition outside. Child 1 (C1) was standing near the staff break room door. C1 fell and was trying to pull himself up, placing his hand inside the hinged side of the door. Staff 3 (S3) was exiting the break room and did not notice C1 hand inside the door. S3 closed the door while C1 hand was still placed in the door causing bruising. First aid was applied, parent was called, and staff was written up. During this inspection, LPA toured the facility and interviewed staff and spoke with parent. Based on the information obtained and interviews conducted. S3 acknowledged she accidentally closed child's finger in the door hinge, because she did not see him; therefore, deficiency is being cited in accordance to Title 22 of the California Code of Regulations and/or Health & Safety Codes. 809-D . An exit interview was conducted, a copy of this report, notice of site visit and appeals rights was provided to Director Telma Rojas.

The facility reported incident to CCLD on 2/24/2025. This is a potential risk to the health and safety of children in care.

This requirement is not met as evidence by: During staff interviews, S1 stated C1 was having a hard time napping and kept moving away from his mat. S1 admitted to picking child up and physically placing him on his mat, which poses a potential health and safety risk to children in care.
