What the inspector wrote

On 4/25/2025, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on and close out the Unusual Incident. The incident took place on 2/19/2024 and was reported by telephone to CCLD on 2/24/2025. The incident was not reported in a timely manner (deficiency cited 809-D) The LPA was greeted by Director, Telma Rojas and toured the facility and took a census of the children. LPA observed a total of 81 children in care with 19 staff members. Description of the incident : On 2/19/2025, during nap time between 12:30 to 2:00pm, parent of C1 arrived to the facility to pick child up. Parent was at the door when she observed Staff 1 (S1) physically picking her child up and forcefully placing him on his mat. S1 stated child was on his knees and she placed him on his mat. On 2/20/2025, Parent spoke with Director about what she observed. Director, stated she would do an internal investigation and follow-up with parent regarding the incident. During this investigation, LPA toured the facility and interviewed staff and spoke with parents. Based on the information obtained and interviews conducted. S1 stated she did place the child on his mat a few times to rest. Staff explained she did not forcefully place child on the mat, instead picked child up under his arm and put him on his mat and stated he needed to take a nap; therefore, deficiencies are being cited in accordance to Title 22 of the California Code of Regulations and/or Health & Safety Codes. 809-D . An exit interview was conducted, a copy of this report, notice of site visit and appeals rights was provided to Director Telma Rojas.

The facility reported incident to CCLD on 2/24/2025. This is a potential risk to the health and safety of children in care.

S1 admitted to picking child up and physically placing him on his mat, which poses a potential health and safety risk to children in care.
