On 08/05/2025, Licensing Program Analyst Adrian Risher conducted a case management inspection regarding an incident that occurred on 07/25/2025 . LPA met with the Telma Rojas, Director. LPA observed 68 children present with 19 teachers. On 07/29/2025, the facility reported an incident involving child 1 being hit by child 2 while playing on the playground. During today's inspection, LPA conducted interviews with facility staff and children. Child 1 was not present during this inspection. LPA received a copy of the children's roster and clock out times for staff who worked on 07/25/2025. Based on the investigation which included interviews with relevant parties and observations by the LPA, further investigation is needed. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to
