On 08/28/2025, Licensing Program Analyst Adrian Risher conducted a follow up case management inspection regarding an incident that occurred on 07/25/2025 . LPA met with the Telma Rojas, Director. LPA observed 25 children present with 7 teachers. On 07/29/2025, the facility reported an incident involving child 1 being hit by child 2 while playing on the playground. On 08/29/2025, LPA conducted interviews with facility staff and children. Child 1 was not present during this inspection. LPA received a copy of the children's roster and clock out times for staff who worked on 07/25/2025. Children stated staff stay outside on the playground with the children. Director stated she conducted an investigation regarding the incident. Staff did not observe the alleged incident. Based on the investigation which included interviews with relevant parties and observations by the LPA, there was insufficient evidence to determine that Personal Rights or Lack of Supervision violations occurred. No deficiencies will be cited today. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Telma Rojas, Director.
