What the inspector wrote

On 10/16/2025 Licensing Program Analyst (LPA) Lisa Clayton arrived unannounced at Launch Pad Learning to conduct a Case Management – Incident inspection, following up on an Unusual Incident that occurred on 09/15/2025. LPA Clayton observed 61 children being supervised and cared for appropriately by 18 fingerprint cleared staff. On 09/25/2025 Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced visit and interviewed staff and received documents related to the incident. The Unusual Incident involved an upset parent yelling at staff in the presence of children (for full incident details, see LIC 812 dated 09/15/2025 and LIC 809 dated 09/25/2025). LPA Clayton received written/typed accounts of events involving the parent referenced in the Unusual Incident during todays visit (see facility file). Based on LPA Clayton’s interviews and record review, the parent has exhibited the above-mentioned behavior on at least 2 other occasions. Based on LPA’s interviews and record review, Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, a Type B Citation for Personal Rights Violation is being issued today (see LIC 809D). An exit interview was conducted, the report was reviewed and provided to Assistant Director Violet Ruiz. Appeal Rights were discussed and provided. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.

(2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as eviidenced by: LPA Clayton's interviews and documentation provided by the school which showed that the parent behaved inappropriately in the presence of children in care on at least 2 occasions, which posed a Health and Safety risk to children.
