Incident investigation·April 2, 2026

On 04/02/2026, Licensing Program Analyst Adrian Risher conducted a case management inspection regarding an incident that occurred on 03/10/2026 . LPA met with the Netsanet Lessanework , Director. LPA observed 62 children present with 17 staff. On 03/11/2026, the facility reported an incident involving the allegation that child 1 was hit by staff. On 03/23/2026, LPA conducted interviews with facility staff. LPA Risher received a copy of the facility roster and personnel report. On 04/02/2026, LPA conducted interviews with children. Staff reported child 1 was redirected when she was not following directions during meal time. Children stated they did not observe staff hit child 1. Staff did not interact inappropriately with children in care. Based on the investigation which included interviews with relevant parties and observations by the LPA, there was insufficient evidence to determine that any Personal Rights violations occurred. Child 1 was not hit by staff. No deficiencies will be cited today. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Netsanet Lessanework, Director
